Short Term Disability Benefits

 

Short Term Disability Benefits in general cover weekly benefits payable for non-work related illness or injury beginning on the 1st day of an accident or the 8th day of an illness.

To apply for Short Term Disability Benefits call Union Labor Life Insurance Company at 1-888-855-4261 to file your claim by phone. You will need the following information:

1. Name, Social Security Number, Home Address and Telephone Number
2. Name of your most recent employer and phone number, if available
3. Name and telephone number of your physician(s) and other medical providers
4. Date of last office visit and next office visit, if available
5. Date you last worked
